Your co-payment is due at the time of your
appointment. We accept cash checks, debit and credit cards (VISA
and Mastercard only)
We will file your primary and secondary insurance as a courtesy to
you. You will be responsible for your deductible and coinsurance
as well as any amount over what your insurance doesn't pay when your
insurance is a non-participating payer. If you are unsure of
whether our physicians participate in your insurance plan, please
contact your insurance company for confirmation. |
